Children's Ministry Annual Registration (Returning or Regular Families)

If your child will be participating in Fraser Lands Church Children's Ministry programs throughout the current ministry year (e.g., Sunday School, Kidz Church, and other ongoing children's programs), please complete the Annual Registration Form. This form only needs to be submitted once per ministry year.

Sunday Programs Visitor Form (First-time or Occasional Visitors)

Are you visiting our Children's Sunday Programs as a guest? We'd love to welcome you! Please complete the Visitor Form before attending. This form needs to be filled our per visit.
